Bremen, Germany (Sports Network) - Dutch winger Eljero Elia is on the verge of completing a move to Werder Bremen after passing a medical with the Bundesliga club Sunday.

Elia joins Bremen from Italian side Juventus on the back of the club's undefeated league season and Serie A title triumph last term. Elia joined the Turin club last summer but logged just four appearances, losing his place in the Netherlands squad as a result.

The 25-year-old has will put pen to paper on a four-year contract and the move will be completed officially when Juventus announces the deal on the Italian Stock Exchange on Monday.
